A newly published analysis using the Electronic Frailty index (eFI) identifies the number of older people with severe, moderate and mild frailty in Camden.
The analysis shows that about a third of the older people (65+), registered in the Camden’s GP practices, have a mild frailty and at high risk of fall-related admissions, enabling preventative measures to be put in place.
This work will inform the Clinical Commissioning Group to develop an integrated care pathway for older people with frailty by considering individual vulnerability rather than by age alone.
